Registration for 2021-2022 School Year Dates & Instructions

2021-2022 REGISTRATION INFORMATION

 Dear Parents and Guardians,

The Brookhaven School District will follow the same registration and residency verification process that we conducted for the 2020-2021 school year. We will continue to utilize an appointment schedule at each school site for new student enrollment and returning students who have had a residency change since the 2020-2021 school year.

 Registration Guidelines

 Kindergarten Students Online Pre-Registration

All students entering Kindergarten may register online beginning Monday, March 22nd.

Do you already have an ActiveParent account for other children attending Brookhaven Schools? If yes, you will use the same account for your new Kindergartner. If no, we will create an ActiveParent account and password for you to use.

Please email the information listed below to registration@brookhavenschools.org

  • Parent's name (Must be the parent the child resides with)
  • Parent's email address 
  • Child's full name
  • Child's date of birth

Please allow 24 hours for a response. A 'Quick Guide' will be attached to the return email.

Beginning Monday, July 12th, call Mamie Martin Elementary School to make an appointment for verifying your child’s information and submitting all required registration documentation listed below.

 All Returning Students Whose Address Has Not Changed From the 2020-2021 School Year

Two current proofs of residency will not be required if your address has not changed from the 2020-2021 school year.

  • Online registration begins Monday, June 14th.
  • Login to your Active Parent account and verify your child’s information.
  • Complete and attach the Address Verification Form using the Residency Proof Documents tab. This form is found on our website.

 All Returning Students Whose Address Has Changed From the 2020-2021 School Year

Two current proofs of residency will be required if your address has changed from the 2020-2021 school year.

Beginning Monday, July 19th, call your school site to make an appointment for verifying your child’s information and submitting your current two proofs of residency.

 All New Students

Appointment Protocol

We encourage you to:

  • come alone
  • use hand sanitizer before walking up
  • bring a pen to use
  • practice social distancing
We ask that you wear a mask if you have not received the COVID-19 vaccine.

Please stay home if you are sick or have symptoms of COVID-19, which include a fever, cough, or shortness of breath. We are glad to reschedule an appointment for a later date. 

We are happy to work with you on the best way to turn in the needed documents for registration if you are in a high-risk health category for contracting COVID-19. Please contact us for assistance.

7th Grade Parents: Incoming 7th grade students are required to have a pertussis (Tdap) booster if they have not received a Tdap vaccination since the age of seven. This may be submitted as an attached document during online registration for those whose address has not changed from the 2020-2021 school year.

 Required Documents for Kindergarten and New Student Enrollment: 

The following documents should be submitted to your child's school by appointment only to complete the registration process:

  • Immunization Records (Mississippi Immunization Form 121)
  • Certified Birth Certificate
  • Withdrawal Report
  • Transcript from prior school (high school only)
  • Copy of Social Security Card
  • Medicaid Number (if applicable)
  • Notice of Special Needs
  • Valid Mississippi Driver’s license or Mississippi ID (Parent or Guardian)
  • An “Affidavit of Residence” is required when a student uses the address of an adult other than parents/legal guardians to claim district residency.
  • Two (2) Proofs of Residency - A post office box address will not be accepted. The school may require additional documentation or verification at any time, particularly if there is a discrepancy or a reason to question a student’s listed address. Two (2) of the following items may be used for address verification. One (1) item must come from each list:
